Dandruff and hair loss: is there a link?

Can dandruff make your hair fall out? Here is what the evidence says about flaking, seborrhoeic dermatitis and hair shedding, and how to keep an itchy, flaky scalp under control.

Dandruff is one of the most common scalp complaints in Malaysia, and many people worry that the flakes and itch are making their hair fall out. The relationship is less direct than it seems. This guide explains what dandruff is, how it may be linked to hair shedding and what may help.

What dandruff is

Dandruff is a common condition in which the scalp sheds small white or yellowish flakes of skin. It is often itchy. It is not caused by poor hygiene and is not contagious 1. A more inflamed form, seborrhoeic dermatitis, causes red, greasy, scaly patches on the scalp and sometimes on the face, eyebrows, sides of the nose and chest 2 3.

Both are thought to involve a reaction to a yeast called Malassezia that lives on normal skin, along with oil production and individual sensitivity 4 5. Heat and humidity can make the scalp feel oilier, and many people notice flare-ups with stress or lack of sleep.

Does dandruff cause hair loss?

Dandruff itself does not usually cause lasting hair loss. However:

  • Scratching an itchy scalp can damage hairs and cause breakage.
  • Significant scalp inflammation, as in seborrhoeic dermatitis, may be associated with increased shedding in some people 4.
  • Dandruff and pattern hair loss are both common, so many people have both at the same time. Treating dandruff will not reverse pattern hair loss.

When scalp inflammation is controlled, shedding linked to it often settles.

Conditions that can look like dandruff

Not every flaky scalp is dandruff. See a doctor if your scalp has any of these features, because they need different treatment:

  • Thick, silvery scales that extend beyond the hairline may be scalp psoriasis 6 7.
  • Scaly patches with broken hairs or bald areas, especially in children, may be a fungal infection called tinea capitis (scalp ringworm), which needs oral antifungal medicine 8 9.
  • Red, sore bumps around hair follicles may be folliculitis 10.
  • Redness, scaling and hair loss with scarring, for example in lichen planopilaris, needs prompt specialist care 11.
  • Itching with tiny white specks stuck to the hair may be head lice eggs rather than flakes 12.

How dandruff is usually managed

The NHS and American Academy of Dermatology suggest these steps 1 13:

  • Use an anti-dandruff shampoo. Look for active ingredients such as ketoconazole, zinc pyrithione, selenium sulfide, salicylic acid or coal tar. Follow the instructions on the pack, and leave the shampoo on the scalp for the recommended time before rinsing.
  • Try a different active ingredient if one does not help after a few weeks.
  • Wash regularly. People with oily scalps may need to wash more often.
  • Once it improves, many people can use the medicated shampoo less often to keep it under control.
  • Avoid scratching and harsh scrubbing.

People with darker or tightly curled hair may be advised to wash less often and use a medicated shampoo about once a week 13.

Ketoconazole and hair loss

Ketoconazole shampoo is commonly used for dandruff. Some small studies have looked at it in pattern hair loss, and a systematic review suggests it may have a role, but the evidence is limited and it is not a main treatment for pattern hair loss 14.

When to see a doctor

See a pharmacist or doctor if 1:

  • Your dandruff does not improve after using anti-dandruff shampoo for about a month
  • Your scalp is very itchy, red, swollen or painful
  • You notice bald patches or hair falling out in clumps
  • Flaking spreads to your face or other areas

Choosing products

When you shop for a hair loss shampoo, remember that a shampoo can help manage dandruff and keep the scalp healthy, but there is limited evidence that any shampoo on its own can treat pattern hair loss 15. If you are also noticing thinning, read what to do about hair loss and consider seeing a doctor.

Frequently asked questions

Can dandruff cause hair loss?

Dandruff does not usually cause lasting hair loss, but scratching and significant scalp inflammation may increase shedding or breakage. Shedding linked to inflammation often settles when it is controlled 4.

What shampoo is good for dandruff?

Anti-dandruff shampoos contain active ingredients such as ketoconazole, zinc pyrithione, selenium sulfide, salicylic acid or coal tar. If one does not help after a few weeks, try another 1 13.

Is dandruff caused by poor hygiene?

No. Dandruff is not caused by poor hygiene and is not contagious, although washing regularly may help 1.

When is a flaky scalp not dandruff?

Thick silvery scales, patches with broken hairs, sore bumps or scarring may be psoriasis, fungal infection, folliculitis or another condition and should be checked by a doctor 6 8.
